A small amount of identity context

Before a contribution can be saved, the approved identity step will explain what is requested and why.

Email — required Used to connect the contribution, reduce duplicate responses, and support an approved return path.
First name — optional Used only as a preferred form of address unless another permission is given.

Before you begin

Most sections take about 3–7 minutes; Share Your Story may take up to 8 minutes.

Share only your own experience or a role you can accurately describe.
Do not include account numbers, signatures, financial details, medical information, passwords, or unrelated personal information.
Avoid naming individuals or making detailed legal allegations in open text.

Private by default

Individual answers remain private by default. Public results use eligible aggregates, and written comments are never published automatically.

Contact and publication choices must be captured separately.
Choosing a display preference only makes a comment eligible for review.
Full addresses and supporting files are not shown publicly.
